Bunnell, Florida — Every three years, tax-exempt hospitals and county health departments in Florida are required to conduct Community Health Assessments (CHAs) and develop Community Health Improvement Plans (CHIPs) to address specific opportunities for improved community health. In Flagler and Volusia counties, health department officials collaborate with AdventHealth, Flagler Cares, Halifax Health, SMA Healthcare and county government to develop shared assessments and improvement plans.
The first step of this collaborative process is to survey community stakeholders. Accordingly, residents of Flagler and Volusia are encouraged to participate in the 2021-2022 survey which launches December 1. Community input is an important component of the plan to address community concerns and improve quality of life. It’s important for residents to take the time and let their voice be heard. These voices influence health priorities for the coming years.
The survey is online at countyhealthsurvey.com and is available in English and Spanish. Paper copies of the survey are also available upon request.
Results of the survey provide the foundation of the Community Health Assessment. Once the assessment is complete, it will be used to create three-year Community Health Improvement Plans for both Volusia and Flagler counties.
Residents, employers and community partners are encouraged to share the link and ask others to take the survey. The survey will close January 28, 2022.
